This chart is used to assess the quality of the habitat of the stream. The first column lists "assessment factors," or features of the habitat that are evaluated. The other four columns provide descriptions of those features depending on their condition - whether they are in poor, fair, good or excellent shape. Students decide which boxes to circle for each habitat feature.
